We help people manage long term conditions and provide community nursing, often at home. These services include management of diabetes, bladder and bowel problems, wound care and home oxygen.

We help people avoid admission to hospital with our Rapid Response and Home First teams, and speed up their recovery in specialist community-based rehabilitation wards

We help people with ongoing conditions to retain and improve their independence. These services include: physiotherapy, podiatry, speech and language therapy and occupational therapy.

We help improve children's development and support them to reach their full potential. Services include: audiology, speech and language therapy, occupational therapy, physiotherapy, and specialist community nursing.

We work in partnership with adults with a diagnosis of learning disability, their families, carers, and other agencies to overcome difficulties. Our team includes health and social work professionals.

We offer a range of support and advice services to help you stay well. These include support to stop smoking, and with other ways of improving health.
