The Diabetes Integrated Care Service (DICE) provides specialist care delivered locally in Ealing for people with complex or difficult-to-control diabetes.

This service provides specialist diabetes care for adults in the community, including:   

  • Medical and nursing consultant-led clinics
  • Diabetes specialist nurse clinics
  • Diabetes specialist dietetic service
  • Health promotion events
  • Education programmes for patients and professionals.

We offer community-based support (sometimes called Tier 2 and 3 services). These are mainly for people who have Type 2 diabetes without significant medical complications.

Most patients with Type 1 diabetes and those with complex medical needs are managed by hospital teams.

Our clinics take place across Ealing so that you can access care closer to home. 

Referrals to the service are made via your GP or by referring yourself to Right Start Education which is available on-site or online.

We work together with people with diabetes to agree individualised outcomes, personalised care plans, and discharge plans.


We support people with diabetes to improve their control and to reduce complications by focussing on blood glucose levels, blood pressure, cholesterol, and weight control/management.


Our multi-disciplinary team includes:

  • A Medical Consultant
  • A Diabetes Specialist Nurse Consultant
  • Diabetes Specialist Nurses (DSN): Provides information,support, and advice on diabetes medications including insulin, injectables, and tablets
  • Diabetes Specialist Dietitians (DSD): Provide dietary and nutritional treatment/management plans
  • A Diabetes Specialist Psychologist (DP): Supports patients with emotional and psychological problems related to their diabetes
  • Administrators: Support patients with making appointments and arranging transport and interpreters when needed.

People over 18 years old with Type 2 Diabetes who need specialist care. You might also be seen by us if your case is too complex for your GP practice, but not complex enough to warrant hospital input.

We provide both local face-to-face and remote appointments. Most new appointments will last approximately 45 minutes and follow-up appointments are usually 30 minutes.


During your appointment, a personalised action plan will be agreed between you and the clinician. You will work with the clinician to review your medication, diet, and lifestyle. If necessary, our clinicians can refer you to other appropriate services.

Please bring all your medication, your blood glucose diary, and your glucometer (if you have one) to your first appointment. We will aim to see you at the time of your appointment, however, some appointments may overrun and this can cause a delay in the clinic.


You will need to have a blood test as requested ahead of your review appointments.

If for any reason you cannot attend your appointment, please make sure you contact us to cancel or rearrange as soon as possible.

Right Start is an education programme for people with Type 2 diabetes. It involves our diabetes specialist dietitians, podiatrists and diabetes specialist nurses.

The programme is designed to help you understand your condition and increase your confidence in managing your diabetes. This is a free service if you’re registered with a GP in Ealing.

You can attend the Right Start programme if you:

  • Have Type 2 diabetes
  • Can get to the sites where we provide the programme (we can’t provide transport, but the online programme might be an option)
  • Are able to understand English or Punjabi – we hope to provide the programme in other languages in the future
  • Are registered with a GP in Ealing.
  • Are motivated and able to commit to 4 sessions, which last 2 hours each.
