  • The Community Recovery Service is an integrated health and social care service for adults which supports independence by providing rehabilitation and reablement after an acute illness, injury or change in life circumstances.

  • The Diabetes Service in Hounslow provides support for Diabetes management and specialist support In-GP practice, telephone advice line and email advice service for clinicians, structured education sessions and home visits for housebound clients.

  • We are working with Hestia and the London Borough of Hounslow to provide an Enhanced Dementia Care Service to local residents in the borough.

  • The Hounslow Falls Prevention and Bone Health Service aims to reduce the rate and risk of falls and fragility fractures amongst Hounslow adults, and to improve the health and mobility of older people and sustain their independence.

  • This specialist service accepts referrals for the treatment of hand injuries from Hounslow GPs and local hospitals / walk-in centres.

  • The Integrated Community Response Service (ICRS) aims to prevent people from being admitted to hospital if they don’t need to be. When someone does need to be treated in hospital, we help them to be discharged as soon as possible to continue their care at home.

  • Our intravenous therapy service is an integrated community service led by clinical nurse specialists in intravenous (IV) therapy.

  • The MS Service is provided as part of the Hounslow Community Recovery Service. The service provides specialist therapeutic and practical support to people with MS, their families or carers at the time they need it most.

  • Our adult nutrition and dietetics service is a team of registered dietitians who provide one-to-one telephone, video, face to face and home visit consultations for a wide range of problems including gastrointestinal issues, malnutrition, weight management and home enteral feeding.
