Referrals
Patients can be referred to this service by their GP, or other health or social care professional.
Patients must be registered with a Hounslow GP.
This service is open to adults with diabetes only.
Consultation is by appointment only.
GP referrals should be sent electronically via SystmOne task.
Referral forms can be found in GP SystmOne address book.
GP practices not on SystmOne should email hrch.podiatry@nhs.net
Referrals from other healthcare professionals should be made using the referral forms below and emailed directly to the community diabetes service via hrch.podiatry@nhs.net.
Adults with type 2 diabetes can self-refer for the X-Pert education programme. Please complete and submit the online referral form and we will contact you in due course.
