Single Point of Access is a one-stop referral system for patients residing in the London Borough of Hounslow for the following services:
Integrated Community Response Service
Adult Community Nursing (Community Nursing, Community Matrons, Community Phlebotomy Service, Tissue Viability, Continence Service, Continuing Care Team)
Rehabilitation Services in Hounslow (Community Rehabilitation Service, Neuro-Rehabilitation Team)
The service has been developed to process referrals for several services using a universal referral form, ensuring all essential information is completed - thus reducing time delays and avoiding unnecessary duplication.
Referrals are processed on the day of receipt.
This form is to be completed for all patients who are being discharged back to their own homes or usual place of residence with a new or changed need for community support.
North West London Needs-Based Assessment Form
The North West London Needs-Based Assessment form can be used to refer patients registered with a GP in any of the North West London boroughs for community healthcare support delivered in their homes. For patients registered with a Hounslow GP this includes referrals to:
- Adult community nursing (community nursing, community matrons, community phlebotomy service, tissue viability, continence service and the continuing care team)
- Integrated community response service
- Rehabilitation services (community rehabilitation service and neuro-rehabilitation team).
Completed forms should be sent to the Single Point of Access: hounslowandrichmond.spa@nhs.net
All referrals need to be completed on the Single Point of Access referral form.
Referral forms can then be emailed across to SPA at hounslowandrichmond.spa@nhs.net
