The Assertive outreach is a specialist service that works with people who have a history of serious mental health problems, but are no longer in regular contact with mental health services.
The people we help often have a range of complex needs and we work with less people so we can have more time to support each individual in an intensive and flexible way.
We try to develop and maintain long-term supportive and therapeutic relationships, helping people to maximise their strengths and abilities whilst acknowledging the limitations and problems that mental health issues can create.
Our aim is to deliver anything that is required for people to remain in the community and maintain social inclusion.
What we offer
We focus on the delivery of community support through a team approach, high staff to patient ratio and the use of community resources, such as family, friends, neighbours, employers and voluntary services.
Who is this service for?
People who are registered with a GP (family doctor) in the Tri- Boroughs who have a severe or persistent mental health problem and have difficulty maintaining contact with services.
Referral to this service
Referrals are made from within mental health services by the current care co-ordinator or care team.
