The forensic outreach service provides aftercare and support for men and women aged 18 and over who are returning to the community from forensic inpatient care.
It’s provided for those people who are discharged from inpatient forensic services and for those who are referred from other sources. We’re also involved with the rehabilitation and preparation for discharge for people receiving care in the inpatient forensic services.
Our multidisciplinary and holistic approach helps people with very complex needs to settle back into society and get back to work.
Some people are discharged earlier than they might have been because of the support we can provide in the community. Because we get to know people in their own home setting, we can work proactively with service users to make sure they continue to live safely in the community and prevent re-admission to hospital.
People are referred to this team through the West London Forensic service.
