The winter season can often affect how we feel physically and mentally with shorter days and colder weather. These challenges might make it harder to spend time with family and friends, especially during festive times when we really want to be together. 

For your winter wellness, here is some helpful information about the services we offer to ensure you seek the right support from the right people.

Mental health support

Talking Therapies 

Feeling low, anxious,  depressed, having trouble sleeping? We’re here to help. 

Talking therapies, or psychological therapies, are effective and confidential treatments delivered by fully trained and accredited NHS practitioners.

Available to adults aged 18 or over, contact your GP to be referred or complete the self-referral form online here.


Single Point of Access helpline 

If you're in a mental health crisis you can speak to the Mental Health Single Point of Access at any time of the day, the service is open to all. 

Help is here, just a phone call away, free - 24 hours a day, seven days a week, 365 days a year. You can call us on 0800 328 4444 and 0800 0234 650.


Perinatal services

The perinatal mental health service is a special team that supports women who're having mental health problems when they’re pregnant or after they’ve had their baby. Difficulties in this period are very common. Refer yourself online. Or call the Perinatal MH Team, Monday to Friday, 9am to 5pm

If you have a serious or life-threatening medical emergency, call 999 or go to accident and emergency (A&E). A&E departments are for seriously ill people with life-threatening conditions and will be very busy over winter. We encourage you to use alternative health services if it is not an emergency.

The Circle, a crisis prevention café   

The Circle is a hub space and café that offers mental health support for young people in Ealing and Hounslow who are near, or at a crisis point with their mental health.

Best For You is a website for young people offering a wide range of tried-and-tested digital resources, providing 24/7 support for young people. It also connects organisations within and beyond the NHS to provide early intervention support, clinical assessment, and specialist treatment. 


Kooth 

An online mental wellbeing community providing free, safe, and anonymous support for Children and Young People.

Signing up is free, providing access to a range of mental health and wellbeing support resources including counsellors on chat, and the Kooth podcast, all online and anonymous.

Physical health support

If you're unsure, or have a non-life-threatening medical need, visit 111.nhs.uk or call 111 who can also direct you to your local mental health crisis line if your concerns are related to mental health.

NHS 111 is here to make it easier and quicker for patients to get the right advice or treatment they need, be that for their physical or mental health.

  • 24 hours a day, 7 days a week.
  • To get help from NHS 111, you can:

  • Call 111 for free from a landline or mobile phone.

Please only attend A&E if you have a physical health emergency. 

Urgent Community Response (UCR) Teams are multi-disciplinary teams who respond to a crisis in somebody’s home (including residential and nursing homes), saving them an unnecessary admission to hospital.  They are a crucial service, both in helping people maintain their independence and manage complex and long-term conditions, and in easing the pressure on acute and emergency services.

  • Patients can be referred to UCR Teams by a number of referrers including 999, 111, Emergency Departments, care homes, GPs and primary care providers – and if they feel UCR might be able to support them they can ask via 111.
  • UCR teams can draw on a wide range of skills, including, advanced clinical practitioners, specialist nurses, occupational therapists, physiotherapists, paramedics, therapy assistants and carers. They will respond within two hours of referral.
  • UCR in North West London provides services from 8am - 10pm, 7 days a week, at a minimum. West London NHS Trust has two UCR teams covering Ealing (24/7 via Ealing Community Partners Single Point of Access) and Hammersmith and Fulham (8am – 10pm) via the Community Independence Service Single Point of Access). Hounslow residents can access a similar service known as the Integrated Community and Response Service (ICRS) from our neighbouring Trust - Hounslow and Richmond Community NHS Trust. 

Admission Avoidance Services, Clayponds Hospital - Magnolia Ward, Ealing

The Admission Avoidance Services are part of our ongoing commitment to improve patient care and access to services. The service is designed to provide high quality healthcare to aid patient recovery in a dedicated ward environment for up to 7 days. Falls, chronic obstructive pulmonary disease, heart failure and urinary tract infections are some of the health conditions the specialised nursing team provide care for.

The service includes:

  • A daily medical review with consultant geriatrician oversight
  • A specialist team of therapists offering dietary, Physiotherapy, speech and language support and more. 
  • A team of nursing specialists including access to specialist tissue viability nurse input as required
  • Medicine management
  • All referrals should be made via the Ealing Community Partners Referral Hub by phone on 0300 123 4544.
  • Patients requiring any form of emergency service should be referred to the Rapid Response Teams or acute hospital services (including outside of opening hours 09:00-17:00).

Respiratory Services Hounslow

Hounslow Respiratory service supports service users with pulmonary rehabilitation, home oxygen assessments and reviews, chest physiotherapy and specialist nurse support for chronic obstructive pulmonary disease (COPD) patients discharged from hospital.

  • Referrals for the service will be accepted from primary care and acute hospitals.

Respiratory services Ealing

The Ealing oxygen and respiratory case management team provide respiratory care for people with chronic pulmonary obstructive disease (COPD). The team take a proactive approach to identifying and supporting high risk patients so they can continue living independently in their own home wherever possible.

The service includes:

  • Home oxygen service – assessment and review (HOSAR).
  • Management and specialist support of patients with unstable or more complex respiratory disease.
  • Primary care education and support.
  • Care is provided at home where appropriate or across clinics in Ealing.
